In order to serve you more efficiently, we ask that you bring the following information on your first visit to our office...

   1. Paperwork included with this mailing including (but not limited to) the “Welcome” form, and financial policy.

   2. Any available claims forms and/or identification cards from your dental
insurance provider.

   3. Records including radiographs from your previous dentist. In some cases, our office will request those for you.

   4. A listing of medications you are currently taking, including over the counter drugs and herbal medications.

   5. All parents or guardians are asked to remain in the lobby when your child is called to the back office.

Our friendly staff will invite you back after your child has been seen by Dr. Wise and/ or
the Hygienist.
